- I a student did the private study, a local candidature certificate for that particular 7 years period preceding the year of passing X class from the competent authority in Form Appendix I certificate of residence prescribed vide Sub clause (ii) of clause (a) of para 7 of the Presidential Order (proforma is herewith enclosed in the Official Notification).
- For the candidates migrated from Telangana, they shall submit a certificate of local candidature as per GO No 132 & 133 dt: 13.06.2017.
- In the absence of the suitable certificate, the candidate will be considered as non-local and further proceeding will be done as per rules in force.
- Valid caste certificate copy. In the cases where of a valid caste certificate not submitted, the candidate will be considered as OC.
- Valid EWS (Economically Weaker Sections) certificate copy in case applying in EWS categories.
- Certificate of disability issued in SADAREM.
- The Service certificate to claim the weightage for Contract/outsourcing/honorary service, The Service certificate is to be issued by the controlling officer concerned (DM&HO/DCHS/ Principals of GMCs / Superintendent of GGH / Any competent authority who appointed the applicant). (Service Certificate proforma is enclosed in the official notification)
- If the valid service certificate not submitted, the candidate will not be given service weightage.
Note:- Candidates must submit visible documents, failing which the application will be rejected. Applications without the above documents will be rejected.

1.Weightage up to 15% will be given to the candidates working on Contract /Outsourcing/Honorarium service basis in the Govt. of Andhra Pradesh who are appointed as per ROR in Open Transparent manner including COVID-L9service as shown belowsubject to their Satisfactory service certified by the competent authority
2. As per the G.O.RT.No.10B, HM&FW(E1), Department, dated 1,4.02.2024 consideration of service weightage for the service rendered in the cadre of CHOs working in Village health Clinics for future recruitment of Staff Nurse on contract basis.
3. Weightage to contract employment based on their working area:
(i) @ 2.5 marks for every completion of six months in Tribal Area
(ii) @ 2.0 marks for every completion of six months in Rural Area
(iii) @ 1.0 marks for every completion of six months in Urban areas
Tenure of appointment
The tenure for the Staff Nurse on contract basis is initially one year from the date of joining and it may be extended for further period of time as per the instructions issued by the Government from time to time.
The Selection Committee reserves all the rights to terminate the contract services of any candidate / candidates at any time, with one month notice or as per directions of the Government from time to time.
