1. 10th or its equivalent (To validate date of birth).
2. Certificates of qualifications Degrees as required for the post applied by the candidate.
3. Proof of appearance for the qualifying exam wherever applicable.
4. Marksheets of all years of qualifying exam or its equivalent.
5. In the case where marksheets not available or submitted, marks will be calculated as per applicable rules for the vacancy.
6. A valid certificate of registration in A.P.N.M.C / any other valid council constituted.
7. Study Certificates/ Marksheets from class 4 to 10 from the school where the candidate had studied.
8. In case of the private study, local candidature certificate for that particular 7 years period preceding the year of passing X class from the competent authority in Form Appendix I certificate of residence prescribed vide Sub clause (ii) of clause (a) of para 7 of the Presidential Order (proforma is herewith enclosed in the Official Notification).
9. The candidates who have migrated from Telangana shall submit a certificate of local candidature as per GO No 132 & 133 dt: 13.06.2017.
10. In the absence of the suitable certificate, the candidate will be considered as non-local and further proceeding will be as per rules in force.
11. A Copy of valid caste certificate. In the cases where of a valid caste certificate not submitted, the candidate will be considered as OC.
12. Valid EWS (Economically Weaker Sections) certificate in case of the EWS categories.
13. Certificate of disability issued in SADAREM.
14. Service certificate for claiming weightage for Contract/outsourcing/honorary service, The Service certificate is to be issued by the controlling officer concerned (DM&HO/DCHS/ Principals of GMCs / Superintendent of GGH / Any competent authority who appointed the applicant). (Service Certificate proforma is enclosed in the official notification)
15. If the valid service certificate not submitted, the candidate will not be given service weightage marks.
16. All the above documents must be Self-attested by the candidate.

Tenure of appointment and important conditions

The initial tenure for the Staff Nurse on contract basis is one year from the date of joining and it may be extended for further period, as per the instructions issued by the Government from time to time.

Termination: The Selection Committee reserves all the rights to terminate the contract services of any candidate / candidates at any time with one month prior notice or as per directions of the Government from time to time.
